I live in the Philippines and one of my pugs, a 2 year old male has demodectic mange.  The vets here tried treating him with a wash of  Amitraz, but he had a severe reaction to it within an hour.  He had a staggered gate, acted dizzy, wouldn't sit down, and then he vomited.  I finally washed the dip off of him with a mild shampoo and he recovered within 30 minutes.  
 
We went back to the vet and they then said they would treat him with a shot of Ivermectin every 10 days.  It has been about 5 days since his last shot and he is scratching very aggressively again.  Reading Pug sites on the internet we have tried rubbing him with lemon and that has cleared up the red skin, but I'm sure it isn't killing the mites, just providing some temporary itch relief.
 
The vets here are marginal at best and I've checked with people in surrounding towns and they all use one of the two vets I have taken him to.
 
I have the ability to get some meds from other Asian countries and even the USA. Is there anything you can recommend we can do to treat him more effectively?
